I just didn't take my PPI the day of surgery, and never looked back. The thing is, you are so miserable after surgery that any rebound problems just get rolled up with all the general discomfort that one would hardly notice.
I did cut back to only one dose of PPI. Weaning might be more of an issue for someone on higher doses. Also, if you had a particularly bad experience in getting off the drugs for testing. I definitely had rebound whenever I stopped, but it wasn't so bad as, say, the shoulder pain after surgery, so it would have been hardly noticed.
My sense from talking to the surgeons I interviewed and reading through past boards was that most folks simply stop cold turkey.
