I think many experience heartburn differently. I have had this about two years, but really only recognized it six months ago when I got the classic symptoms of burning behind the breastbone and regurgitation. The vagus nerve that runs up and down one's esophagus branches throughout the body. When my heartburn is really bad I will get burning sensations in my ears, shoulders, upper back, and legs. Had burning in my ears for a long while before I recognized it as heartburn and now my ears only burn when my chest is burning too. Sometimes I think that substernal burning is one of the last symptoms to manifest in GERD.
